To register your athlete at Fishers Junior High School for the 2026–27 school year, please complete the following steps: 1. Create FinalForms Accounts Fishers Junior High uses FinalForms (the same platform used by the high schools) for all athletic registration. IF you have an account from last school year sign in, and upload your new physical. 2. How to Create an Account o Visit our athletics website: fjhathletics.com o Click on “FinalForms” o Under the Parent section, click the blue button labeled “New Account” o Follow the prompts to create and complete both accounts o Be sure to select all sports your athlete will or may want to participate in o Complete and sign all required forms 3. Physical Form Make sure to upload your most recent physical for the final step. You are only required to upload pg.3. This is the page with the doctor’s signature on it. The doctor's signature page must be dated after April 1, 2026. This will be approved by AD Rocky Pollock, then your athlete will be ready to participate!
